Once you’ve completed your 15-minute consultation and you agree it feels like a good fit, the next steps are simple.

Complete Your Initial Paperwork

You’ll receive your paperwork electronically by email.

This includes:

Practice policies and informed consent

HIPPA and privacy information
-
Demographics, and insurance information

Please complete these forms before your first appointment so your therapist can come prepared to meet your needs.

Once the paperwork is complete, your therapist will email directions to the office and if you opted to receive reminders you will receive that notification.

Your First Appointment

Your first appointment is where you and your therapist will have more time to get to know one another.

There will be some information gathering, including things like:

Physical and emotional health
-
Current symptoms or concerns
-
Family and other significant relationships
-
Work, school, or other important areas of your life

Treatment goals
-

You don’t need to have everything figured out or know exactly what to say. We’ll take the time to listen and work through it together.
At the end of the session, we will discuss the frequency of appointments and have the opportunity to give feedback on how the first appointment felt.
